What Is It?
"Take Control of Your Life" is not just therapy — it’s a full-service recovery roadmap tailored for clients navigating complex challenges such as homelessness, unemployment, trauma, parenting breakdowns, or domestic violence. This program combines clinical care with real-world planning to help participants rebuild their lives from the ground up.

What’s Included:
Clients enrolled in the program receive access to a structured plan of care that includes:

• Telehealth Therapy & Weekly Check-Ins
  Supportive 1:1 psychotherapy with licensed clinicians and targeted follow-ups to track emotional growth.
• Basic Skills Training (BST)
  Learn how to manage time, handle responsibilities, and build consistent routines.
• Psychosocial Rehabilitation (PSR)
  Receive social and emotional tools to improve communication, manage symptoms, and set long-term goals.
• Crisis Support
  Immediate interventions for clients in distress, high-risk environments, or housing instability.
• Life Restoration Checklist
  A Medicaid-compliant guide covering:
  • Housing placement and rental navigation
  • Employment readiness and job placement
  • Obtaining ID, bank accounts, and benefits
  • Transportation planning and support
  • Food resources with access times
  • Parenting support and family therapy
  • Domestic violence intervention
  • School readiness and behavior supports for children

Upon enrollment, each client will receive a welcome packet and explainer video. Within 48 hours, a staff member will reach out to confirm details and begin building the individual’s plan of care. All services are delivered via telehealth, with options for asynchronous participation through our secure client portal.
